Home and Community Care Assessment Program

Home and Community Care Services enable clients with disabilities, chronic or acute illnesses to receive needed care in their homes and communities. These services are delivered primarily by the Heskw’en’scutxe Health Services Nurse and Home Care Aides. To receive services, the nurse must complete the assessment in the clients home.

The home care assessment takes approximately two hours to complete. The assessment includes questions about the client’s health history, medication, home environment, and their ability to manage personal care and household tasks.

The assessment process can involve the client, family, and other caregivers. The goal of the assessment is to determine the client’s needs and services required. Following the assessment, the Nurse works with the client and their family and/or caregivers to develop an individualized care plan.

 

The care plan includes services, which are to be provided by the  Nurse and Community Health Care Aides. The care plan is updated as the client’s situation changes. Every year, the client’s needs and the home care community services required are re-assessed by the Nurse and, if necessary, a new care plan is developed.

Home Care Community Services

  • Chronic Disease Management
    Offering ongoing support to assist clients impacted by a chronic health condition (e.g. arthritis, asthma, brain injury, chronic pain, diabetes, high blood pressure, heart disease, kidney disease, lung disease, and stroke). Share the knowledge, skills, and resources needed to better manage on a day to day basis including consultations with any primary care providers and other health care professionals. Assistance with managing your disease by monitoring for any changes, working with other health care professionals, including your family physician and pharmacist and providing useful information and resources.
  • Medication administration and management
  • Wound, ostomy, and foley catheter care
  • Post-hospital care
  • Rehabilitative care
  • Palliative care
  • Medical equipment and supplies
  • Referral to a registered dietician, occupational therapist, or physiotherapist
  • Referral to assessment for publicly funded assisted-living and long-term care home placement
  • Access to medical equipment and supplies (e.g. blood pressure, incontinence products, cane, walker, suction grab bars, and shower chair)

Community Care Aide Program

Tasks delegated by the nurse:

  • Personal care (e.g. bathing, skin care, dressing, the application of compression stockings, and toileting)
  • Homemaking (making the bed, cleaning the bathroom, washing dishes, wiping countertops, dusting, sweeping and mopping, vacuuming, laundry, and taking out the garbage)
  • Meal services (preparation of meals in the home or meal delivery)
  • Transportation (medical transportation or transportation for groceries and supplies)
